Episode 117: How Chronic Stress Drives Autoimmune Disease (The Science Most Doctors Miss)

You’ve probably been told at some point to “manage your stress.” But what does that actually mean, and why does chronic stress have such a powerful effect on the immune system? In this episode of The Goode Health Podcast, we break down the science behind the connection between stress and autoimmune disease, and why it’s one of the most overlooked drivers of immune dysfunction in modern medicine.

Drawing on decades of research in psychoneuroimmunology, we explore how chronic stress changes immune gene expression, disrupts the HPA axis, and increases gut permeability. These are three biological mechanisms that can trigger autoimmune activity and worsen disease flares. Whether you’re dealing with Hashimoto’s thyroiditis, lupus, rheumatoid arthritis, multiple sclerosis, inflammatory bowel disease, or psoriasis, understanding how stress affects your immune system can fundamentally change how you approach treatment and recovery.

This episode answers questions like:

“Can stress cause autoimmune disease?”

“Why stress triggers autoimmune flares”

“Stress hormones and immune cell behavior”

“How chronic stress changes gene expression (epigenetics and immune activation)”

“How does stress damage the gut lining?”

“Why medications alone don’t stop autoimmune disease progression”

You’ll also hear real clinical cases showing how addressing stress physiology, adrenal function, gut health, and inflammation can improve symptom patterns and reduce autoimmune flares. If you’ve ever noticed your symptoms worsen during stressful periods or felt like your treatment plan is missing something important, this episode will help you understand what’s happening inside your body, and why addressing chronic stress is a critical part of autoimmune care. This episode also includes an invitation to the Goode Health Studio.

One of the most frustrating things patients with autoimmune disease hear is ‘you need to manage your stress.’ The problem is that this advice usually comes without any explanation of what stress actually does inside the body. Chronic stress doesn’t just affect your mindset. It changes physiology. It alters cortisol regulation, damages the gut barrier, depletes key nutrients, and drives inflammatory signaling in the immune system. Without addressing those biological consequences, telling someone to manage their stress is like giving them a direction without a map.

Essential learnings from this episode…

  • Decades of research show that chronic stress alters immune function, increasing inflammatory signaling and reducing immune regulation. This imbalance is one of the key drivers behind autoimmune disease development and flare-ups.

  • Under chronic stress, immune cells shift toward pro-inflammatory gene activity while suppressing antiviral responses. This means the immune system can become more inflammatory while also less effective at protecting the body.

  • Chronic stress disrupts the HPA axis and cortisol rhythm, which normally helps regulate inflammation. When cortisol signaling becomes dysregulated, the immune system becomes more prone to inflammation and autoimmune activity.

  • One of the key mechanisms behind autoimmunity is increased intestinal permeability (“leaky gut”). Chronic stress hormones can weaken the gut barrier, allowing microbes and food antigens to trigger immune activation and autoimmune responses.

  • Immune cells have receptors for cortisol, adrenaline, and other stress hormones. Chronic stress can suppress regulatory immune cells while promoting inflammatory cytokines, increasing the likelihood of autoimmune flares and disease progression.

  • Reducing stress is not just about relaxation techniques. Supporting HPA axis health, nutrient status, gut integrity, and inflammation is essential for improving immune regulation and reducing autoimmune symptoms.
